That doesn't look like the default GP comments form, so I think you might have a plugin installed that's adding it and not respecting your settings.

Well, I deactivated all the non-GP plugins, but the pingbacks were still there. I activated a different theme, and the pingbacks disappeared.
Hmm, any changes made to the theme? Using a child theme maybe?

Nope, no child theme. This is getting weird.
What different theme did you activate? Were plugins still deactivated?
I'm afraid I might need to actually see the live site instead of a screenshot to know for sure what's going on.

Make sure both checkboxes are unchecked.
What happens if you disable beaver builder?
Yes, I have done all of that.
I just deactivated all the non GP plugins with the settings you just mentioned, and the pingbacks are still there.
