Can you go through All of your header elements and make sure the Navigation Location under the Site Header tag is set to Default?

I still believe David nailed the issue in his initial reply.

    Hi Leo,

Thanks for clarifying David’s solution. It turns out I had to go through ALL the header elements and set Navigation Location under the Site Header tag to Default.

That seems to have fixed the issue.

    Thanks again!

    Glad to hear that worked.

This also indicates that you might have some conflicts in display rules going on:

So if you run into an issue with elements not displaying or not displaying correctly, that would likely be the reason.
