If I look in the back-end the video looks normal and I can play it. I looked at the URL and i looks correct: https://www.youtube.com/watch?v=4R2taNKqHzU&feature=youtu.be&ab_channel=TheReadyState

I tried your YT link and it works fine on my test site.
Can you try temporarily disabling all other plugins to see if there is a conflict ?

Now I have tried to deactivate all plug-ins, remove my custom code in functions.php, deactivate the child theme and activate the original, nothing works :(

Odd... can you try adding your own Embed code for the video.
This site will create the emebd code for you - and you just need to add it to a HTML Block.

Its really odd that the WP block isn't working correctly and that we cannot replicate it. I'll have a look around on Gutenberg GitHub to see if there is any reason for that. But i am glad to hear that works for you!

I have set a video as HTML block but it is still not showing up :(

Try disabling the lazy loader to see if that is affecting the display of that video
