Portfolio Site: missing contact form (HappyForms-free)

Good morning everyone,
I am a new WP user and I downloaded the Portfolio site from the Library. But the contact form as shown on the template (bottom of page with input fields) did not show up. I don't want to try to recreate it manually if I don't have to. Does anyone know why it might not be appearing?
The plugins are activated.
I don't have any other plugins except the default that come with WP installation.
I tried removing the portfolio template and redownloading it but that hasn't fixed the problem.
Any ideas?

Hi Brian,
Go to appearance > elements > footer, check if the form shortcode is correct.
You should be able to see the correct shortcode in the form plugin's settings.

The shortcode in my Appearance/Elements/Footer seems to be "[happyforms id="3538" /]"
But I don't see where I can find the shortcode of the template. I would attach screenshots but it seems that this forum doesn't allow images except as a url link.
In my dashboard, if I go to Forms/Settings I just get a popup asking me to sign up for a free 7-day trial (I assume this is to upgrade from free to a paid premium version of the plugin). When I cancel out of the popup there are no forms found.

If there's no form, then the shortcode is useless.
In this case, you'll need to create a new form, then replace the shortcode in the footer with the new shortcode generated.
Thanks Ying. I understand now that there needs to be an existing form, and that the shortcodes need to match. I didn't realize that the template wouldn't create its own form for me. But no problem. I can see how to do it manually and duplicate what was done on the template. Thanks for the help.
