How are you using a profile picture in your custom email address?

So, I checked that when you guys send an email, your custom email is using a profile picture, can you tell me how you did it, it is important if you can help me...
Can you tell me how did you setup for the email please?
We use a CRM system ( Helpscout ) for all our email replies, and that handles Gravatar.
