I had a glance over the functions and nothing jumped out as an issue or something that may conflict.
Can you try adding the snippet we provided as the first snippet in your functions.php ?

meanwhile, I move the site to a new WP installation, added the filter that David suggested initially and voilà the issue is fixed ;-).
