Gift Aid checkbox

Hi hopefully I can get what I'm after across to you guys.
Here in the UK, Charities can claim Gift Aid on certain purchases.
What I'm after is a checkbox that will show on checkout but only on a certain product.
This checkbox will say do you want to agree to giftaid this purchase.
I'll then need a means of storing this response along with the customers details.
Any ideas?
I've tried Advanced Custom Fields and some php code which will display the checkbox but not on a specific product.
Any help would be greatfully received

Hi there,
that is going to require custom development - i had a look around and the only plugin for this is out of date and a few forum posts have said its no longer functional.
I did find this feature request though:
The user who replied to the request provides his current workaround.
