Form to Post submit and format?

Hello,

I have website that has every page with the same formating:

Title
Introduction ~ 200 words
3 Product images with links below
Another ~500 words block
YouTube embeded video
10 links

All that is formated in WP editor using some shortcodes from Shortcodes Ultimate plugin.

My writers always send me article in .DOC file and then I need to copy page and do all formating.

What I want to do is to give some "secret" (private) URL with Form that he can fill so all these blocks come into the place inside my design of wordpress.

So, he would enter in different fields of form Title, Introduction, 3 product images, etc...
and "somhow" I need it to go into my formating and shortecodes and to become published post (or page).

Do you know is there any WP plugin for this?
