Form on top of a picture or video

Dear Tom,
How can I create a form (an opt in) on top of a picture or video? Say I have a hero image and I want an opt in on top of that. Is there a plug in your recommend for that or what would be a good idea?

You could use the Page Header add-on to set a background image or video, then add your form HTML into the content area.
Otherwise you'll need to use some custom HTML and CSS within your content.
