I am trying to add an html block and then added the CRM form script.

Can you try adding these two scripts through a Hook Element, and then hook it to after_header?

Can you also help with the footer? Template Footer is missing and I am seeing another footer (maybe the default footer)

Yes, I mean the script for the Form, and the one at the bottom.

For the Footer, there’s no display rule set. Go to Appearance > Elements > Global Footer > Display Rules, and set a display rule location setting.
