Convertbox embed form not showing up

Hi there,
I'm trying to get a Convertbox embedded form to show up on the homepage of my site here in the top hero: http://staging.lifehealthhq.com/
I added the necessary header code using the Hook element, and then added the HTML form as a custom HTML element to the homepage, but it's not showing up.

How did you add the HTML code? It's an empty<div> element in front end.
Can you take a screenshot of your setup?

Hi there - I am using the custom HTML block and adding in the code ConvertBox gave me: < div id="cbox-1kxGMs4Orm8ubltR" >
. (note: I added extra space because it wasn't showing without them in this editor box)
I also added this code to my header (or tried to):

I can see the JS has been added to the site.
But I also see some console regarding the script:

I would recommend reaching out to the form support to fix the script.
