Contact Form

Hi,
I would like to add a form on the contact page (as well as the usual contact details like telephone number, email etc).
When they use the contact form I would like them to answer some multiple choice questions so I can have a basic profile of their business, ie:
Are you a) A Sole Trader
b) A Partnership
c) Charity/Non-Profit
How often will you require this service?
a) One time
b) Weekly
c) Monthly
d) Quartlery
e) Annually
Etc......
Can you please point me in the right direction how I do this.

You would need a plugin like Ninja Forms.
There are quite a few good ones out there. You might have to do a quick research and try out a few to see which one suits you the best.

Thanks - I've used WPforms and it works great.
Just one question - I'm trying to test it out but I'm not receiving the emails (I've added my email address in the send to and from settings).
