Comment Form appears - why?

For some reason with everything disabled, I still get a comment form on only one page!!!!
I have done everything mentioned in this link: https://docs.generatepress.com/article/removing-comments/
I have not added any css to hide it since I shouldn't have to and if we want comments in the future then...

It is only this page. very strange. Can I do something in the database?

That's not the default WordPress comment form, so the WP settings won't remove it.
It seems to be posting to this URL: catapult.fevercom.com
Look familiar? Maybe a plugin or custom script you've added to the site?
