Checkboxes in all Forms disappeared (contact, woocommerce registration etc.)

Hi there.
Today our checkboxes disappeared on two pages with forms on the page. On two other pages with a form and checkbox they didn't disappear. We are not sure if any plugin update is the reason. Can you please help us out urgently? Thanks a lot!

This cannot be a GP issue.
Any chance you can disable all plugins except GP Premium to eliminate conflicts from other plugins first?
