Adding Subscription Checkbox to end of Single Posts

I would like to add a check box at the bottom of each single post with a label that says
Please inform me of all new posts on this site - or similar.
I would like to do this with php and css rather than a plug in.
At the same time I would like to remove the check box that says:
Save my name, email, and website in this browser for the next time I comment.
Please how might this be done?

- That would require more than just the message as there would need to be some sort of custom solution to track when that option is checked. Unfortunately that is outside the scope of this forum.
Have you found a plugin that can do this? It would be your best bet.
