Does the plugin not work?

This is the one we usually suggest: https://wordpress.org/plugins/fancier-author-box/

Fancier Authorbox hasn't been updated for more than two years and so I decided not to use it. The other one has more than 20.000 installations and I will give it a chance. Everything works, but not the code from Tom :-) I started a thread in their supportforum.

If you are using a plugin then the code shouldn't be needed.

What's the instruction from the plugin documentation to implement it with the theme?
