Trouble with Amazon Pay and Checkout Layout

Here's the User Flow creating the problem shown in the following screenshot

1. User clicks book consult: https://www.sandandsteelfitness.com/product/gym-tour-consult/
2. User picks a booking time, and then checkouts out with Amazon Pay.
3. Email address label is the wrong place on the checkout page.

I also tried this flow with the GP Woocommerce addon disabled, and the checkout page looks even worse. Additionally, the normal checkout with Amazon with GP Woocommerce Addon Disabled has serious layout problems (everything is in the wrong place.) Checkout with Amazon (for a product with a cost) with GP Addon enabled looks fine.
