Three column header

Hi,

I am trying to create a three column header:

I've followed the instructions but when I create the Header element, I don't see any checkbox or setting for "Disable Site Header." Also, the navigation is still above the hooked header.

I'm trying to achieve this layout. https://www.avenidental.com/
