Slideout menu is blank - used to work

The slideout menu shows no menu items when it slides out...it's just blank. As you can see, I have three menu items which I believe should appear.
In addition, the little hamburger and word "Menu" stays visible even when I widen the browser, only to disappear if I refresh the page.
Can you advise?

Thank you Leo, a menu was not assigned. As for the enlarging/shrinking menu titling, I see it working for you, though it was doing something other than that for me but now I can't reproduce it.
