I would like to put a registration form on my website. However, this is not a registration form for a newsletter. I am looking for a program or plugin that gives the possibility to a website visitor to fill in multiple open fields. So that in this way I immediately have all the urgent information from a website visitor. So I am looking for a form that works in the same way as google docs forms.

I'm not sure which plugin would fit your specific requirement.
Maybe Ninja forms? Gravity forms?
Might be best to Google a little bit and try a few to see which one works for you the best.
