Sell services and accept payment form

I recently purchased and installed GP Premium and want to add a sell/payment form that is compatible with the Artisan site theme. I have a small service business and would like to add a form that offers a few options for different services, accepts payments, and sends receipt.

I feel like this should be simple but have found nothing that really fits my needs....

I have looked at Stripe, Gravity Forms, etc...I would like a form like the one on the main page of https://www.thinkmaids.com/

Any recommendations would be appreciated.
