Plugin for Media Library

Hello all,
I'm looking for a plugin that allows to upload and manage media in the folders I want, not just the wp-content/upload folder.

Does it exist?
I've read about Enhaced Media Library and others but don't seem to fit my needs...

Hmm, I haven't used any plugins like that - perhaps someone else will have an idea :)
