For now my interest is how to create the fields for name email etc and then provide a link to download materials. Graphic part I'll focus later on.

You will need a contact form plugin like HappyForms, Contact Forms 7 or Ninja Forms.

Then you can consider adding it using the header element.

the Free forms plugins will do the job. Happy Forms is really good for creating simple forms.
