How to add star rating on comment author meta like this

Hi, is possible to add star rating on comment author meta like this one: http://prntscr.com/onj509
I really want to add star rating like that on my site.
I want visitor submit review through review forms below blog post.
I dont want to use any review plugin for this.
So, perhaps I will remove the comment form section and change it into Contact Form 7 form like the site example. (I input the URL of the example site on the "your website URL" form so that you can check it direcly)

Hi there,
To make this alteration to the comments form would require custom development which is outside of our scope.
i wouldn't be adverse to using a plugin, code is code at the end of the day so a well coded plugin will do the same as what you will employ a developer to do.
