Gravity Form won't display in sidebar

For some reason my Gravity Form will not display in the sidebar widget area.
It will display if I insert it into a post or a page, however.

Hmm not seeing any specific errors.
Can you confirm that you are using the latest free theme version 2.1.4?
If so I would recommend trying #1 here to eliminate any plugin conflicts:

If that doesn't help then can you check with Gravity Form's support and see if they have any ideas?

I'm actually using the most recent Premium version, but I turned on WP_Debug mode and I'm seeing PHP errors in the GF plugin folder. I submitted a support ticket with them. Thanks.
