Edit GeneratePress Form

Hi, I've created a form using the GeneratePress forms (which is great!) A couple of problems though:
1) I tried to add an extra column for "phone number" and have to duplicate either the email address form or subject form. The email address only lets you type in an email address so that doesn't work and the subject then says "Enter your subject" inside the box which I try to type over but it doesn't display.
2) My contact form is automatically set up with my email address, but I don't think this form is successfully linked because when I tried to test the form, I don't know whether it was because of the issue mentioned in number 1, but when I try to submit it shows up with an error message. Have I missed a key part of set up?

P.S. I don't know how to get a URL of an image so I'm unable to attach an image to show you more clearly - I can email it over though?

Sorry what is GeneratePress form? We don't have that product.
It's one of the "Ultimate Add On Blocks". I'm on the page builder, I press browse all, scroll down to Ultimate Add On Blocks (UG), and it's in blue so I thought that was from Generate Press? It wasn't available before I downloaded the theme anyway! This particular one is "Forms" and it says "This block allows you to create interactive contact forms, newsletters, suggestions, etc, easily." - Is that not with you?
