Choosing Header Element for Specific Page

OK, I'm tearing my hair out here! I've created a header element, but I only want it to appear on one specific page. However, when I go to display rules and select 'Page' it immediately adds the element to every page of the site. How do I choose just one page? Perhaps it's in the drop-downs, but I don't see it (the drop-downs are hard to read — they're very narrow).
I've put a video in the website URL field for staff to view.

We haven't had this reported before so I would it's either:
- A conflict with an extension from your browser. What browser are you using? Can you test with incognito mode?
- A conflict with another plugin. You can try going through #1 here:

Looks like it was a conflict with the 'Toolset Types' plugin by OnTheGoSystems. I've deactivated it for now. I'll have to ask my client how/why they were using this plugin to see if I can just leave it deactivated.
