Can't HTML code in a right sidebar widget

I use Zoho CRM and Zoho CRM Forms, for visitors to fill in and request a training quote. The code works fine on the web page: https://mindiq.com/contact-us-training-onsite-tailored-low-cost/
However, when I put the code in the Custom HTML Widget, it throws errors and doesn't show up. They offer three versions of the code to use. None work. Unfortunately when I put the code samples here and I submit the ticket, GP security does not let it through. Can you help me here, please?

P.S. Happy to put the code in a Word doc or something to get it through.

Can you test it with a twenty-series theme and see if it works?
That will tell us if it's a problem with GP or the code itself.
